Job Description

Job Duties: The Case Manager will be working under the direct supervision of the Case Manager Supervisor who will oversee their training and performance. The Case Manager will travel from their respective home to the client's home would be compensated at the rate of fifty cents per mile. Each Case Manager is provided with a laptop and will be involved with ongoing training. The Case Manager will have a caseload assigned to them of homebound and Senior Center clients.
