PRN Case Manager
Job Description:
Please note traveling/court/case management/interaction with our external stakeholders is all part of the job. PRN CM will be assigned a County Supervisor to provide guidance as they are assigned a caseload/case. Caseload averaging max 15 cases.
PRN CM will have access to our Documentation System (SHINES) within 15 days date of hire. In the time being, PRN CM will document all contacts in a word document and submit to their assigned Supervisor. The Supervisor will enter said notes into the system.
PRN CM will receive a state issued Laptop/Tablet, State Badge to have access in/out the building, State Cell Phone, assigned office workspace
Job Responsibilities:
Will be assigned a maximum of 15 cases
Will be responsible for making field visits to complete safety assessments
Will make safety decisions on a team approach with the assigned supervisor
Will be completing the components of the investigation per policy and procedures
Will ensure safe case closures
Job Requirements:
Bachelor' s degree in social work related field
Must have license or certification
Completion of supervised clinical experience
Key Skills: listening, organization, interpersonal, compassion, problem-solving, and time management
Computer Skills: Microsoft Office Suite (including Excel), database, desktop publishing, presentation, and medical software programs
Required Documentation and screenings: Standard
